Is Spinal Decompression Painful? What to Expect During Your First Session

If you’ve been struggling with a herniated disc or chronic sciatica, you’ve probably heard about spinal decompression. Maybe you’ve even seen pictures of the specialized table and thought, “That looks like something out of a medieval movie.”

It’s the #1 question we get at Twin Cities Chiropractic: “Is spinal decompression going to hurt?”

I get it. When you’re already in pain, the last thing you want to do is sign up for a treatment that might make it worse. You’re protective of your back, and the idea of a machine “stretching” your spine sounds a little intense.

As a chiropractor who has been using this technology in St. Paul for over 20 years, I want to set the record straight. Not only is spinal decompression not painful, but for many of our patients, it’s the most relaxing part of their entire week.

Let’s pull back the curtain on what spinal decompression actually feels like, why it’s different from a standard “stretch,” and what you can expect when you walk into our office for your first session.

The Short Answer: No, It Doesn’t Hurt

Let’s start with the bottom line: Spinal decompression should never be painful.

In fact, if you feel sharp pain during the treatment, something is wrong. The goal of the therapy is to coax your muscles into relaxing so we can get to the discs underneath. If the machine were causing pain, your muscles would “guard” and tighten up, which would completely defeat the purpose of the session.

Most of our patients describe the sensation as a “deep, gentle pull” or a “distraction” in their lower back or neck. It’s a very rhythmic, slow feeling. Honestly, it’s so soothing that I’d say about 50% of my patients actually fall asleep during their 20-minute session.

How It Works (And Why It Feels Good)

To understand why it doesn’t hurt, you have to understand the technology. This isn’t just a rope pulling on your waist.

At our office, we use a computerized decompression table. When you lie down, the computer calculates exactly how much force is needed based on your body weight and the severity of your injury.

But here is the “magic” part: The computer is incredibly sensitive. It can sense when your muscles start to tense up. If it feels your muscles resisting the stretch, it immediately adjusts the tension to keep you relaxed. This allows us to create negative pressure inside your spinal discs.

Think of a herniated disc like a jelly donut that’s been stepped on. The “jelly” is bulging out and hitting a nerve. Spinal decompression creates a vacuum effect that helps “suck” that bulging material back into the center of the disc. It also pulls in fresh blood, oxygen, and nutrients that the disc needs to heal.

When that pressure finally leaves the nerve, the relief can be almost instant.

A Walkthrough of Your First Session

If you’re nervous about your first visit, here is exactly how it goes down at Twin Cities Chiropractic:

1. The Setup

You don’t need to change into a gown or take off your clothes. We just ask that you wear comfortable, loose-fitting clothing (athletic wear is perfect). You’ll lie down on your back on our specialized table.

2. The Harness

We will fit you with two comfortable harnesses—one around your hips and one around your lower ribs. These are designed to distribute the pull evenly so there are no “pressure points” on your skin. They feel secure, like a firm hug.

3. The Custom Settings

Dr. Sorum will input your specific parameters into the computer. We start very conservatively on the first day. We want to see how your body responds before we increase the tension in future sessions.

4. The Treatment (20 Minutes)

Once the machine starts, you’ll feel a slow, gentle pull that lasts for about a minute, followed by a period of relaxation. This cycle repeats for about 20 minutes. We give you a “safety switch” to hold in your hand, so if you feel even the slightest bit of discomfort, you can stop the machine instantly. (In 25 years, I can count on one hand how many times a patient has actually used it!)

5. Post-Treatment

After the session, we don’t just send you out the door. We often follow up with some gentle massage therapy from Joy Vang or some ice therapy to calm the area down.

Are There Any Side Effects?

While the treatment itself isn’t painful, you might feel a little “different” afterward.

The most common side effect is a bit of muscle soreness, similar to how you feel after a good workout at the gym. Remember, we are stretching muscles and ligaments that may have been tight and stuck for years. It’s very common to feel a little “loose” or slightly achy for an hour or two after your first couple of sessions as your body adjusts to its new alignment.

We recommend drinking plenty of water and taking it easy for the rest of the day after your first few treatments.

Why Choose Decompression Over Surgery?

The reason we are so passionate about spinal decompression is that it offers a way out for people who think surgery is their only option.

Back surgery is invasive, expensive, and requires weeks of painful recovery. Spinal decompression is non-invasive, has zero downtime, and costs a fraction of what a surgical procedure does.
